Forms For Step-parent adoption
I have 3 children from a previous marriage that wish to be adopted by their step-dad. The biological father is willing to give consent. as a matter of fact, he wants nothing to do with the three kids. Does anyone know where I can obtain the paperwork for step-parent adoption? We don't have the money to pay a lawyer, and would like to do the adoption by ourselves. My 3 children adore their step-dad, and have already had a legal last name change done, at their request. So, they have their step-dad's last name, but want the whole package. I have talked to the local court, they won't give out information on what forms I need. I have called printing companies that sell legal documents, none have step-parent adoption kits or forms. I am beating my head into a brick wall here, and have three kids that really want to be adopted. So much, in fact, they wrote a poem asking their step-dad to adopt them and gave him it on fathers day. HELP PLEASE!!

My husband is adopting my daughter. this is what i have found out. Visit this web site, it has most of the forms. http://courts.michigan.gov/scao/courtforms/index.htm you will need, i do believe for each child, forms: PCA 301 PCA 308 PCA 314 PCA 319 PCA 321 you may not need all of them. I don't know what county you live in, but Clinton county tells me that they can guild us in this, but they can not give us legal advice. which means that they should be able to at least tell you what forms you need. My husband is in the process of adopting my 2 children. I was wondering how you got the legal last name change without the bio-parent? You can get the papers you need from the probate court. They also have kits avaliable on-line. You have to do a search for step-parent adoption kits.

I have 3 daughters who want to be adopted by my husband. My ex is all for it. As a matter of fact he is paying for it. He wants to sign off and we told him that he had to pay for the adoption. The main reason he wants to sign off is so he can get out of child support. He picked up the forms from the probate court. We have to file each child seperate but they will all be one case. The filing fee is $160 per child, $30 for fingerprinting of adopting parent, and $26 per birth certificate. This is in Antrim county. We are not using a lawyer. Hope this helps some.
