Need help with adult adoption

Can someone please help me to find the laws and procedures on adult adoptioin. The woman he has become my "mother" since I was 18 wants to adopt me. I want the same thing. I have been in foster care for a good deal of my life and this is a true blessing to have someone take me in as their own at my age. I am now 22 and am still looking for the rules. The other part of this is that I have rebuilt a relationship with my father and so therefore I want my "foster mother" to be my mom and for my father to stay my father and I still carry his name.
