Social security card

Hello,
I am trying to apply for a social security card for DD. Is it enough to show the birth certificate, or is there something else I should bring? Should I bring her hospital records from her birth?

The million dollar question - "What should I bring?"
My impression, from reading others' responses (and from my own), is that you should bring as much as possible. I don't know about the hospital records, though.

I would take the following:
Birth certificate (if you have the amended one, bring that and the original)
Passport (if you have one)
Immunization record
Final adoption decree

The requirements seem to be all over the place, depending upon where you live, what time it is, how upset the attendant is, and what color the sky is that day. Best to just take everything, and see what happens.

Oh Fadzi that was too funny!

I made my DH go and I think he took our adoption decree, new BC and insurance card maybe. Whatever it was, they were in a good mood and took it.

I was kind of worried to get this, based on things I've heard from others. But- the clerk that day was really nice & the whole thing took about 15 min.

I'm with sbaglia- take everything that you have. That way- if they say they need it you've got it. We needed the BC, final adoption decree & my ID. I also took passport, my adoption book (that had paperwork for EVERYTHING- home study, payment, hospital information, etc...),& a letter from the attorney's office.

4/08: application submitted with agency
6/25/08 - officially waiting
6/28/08 - Matched!!
6/28/08 - it's a girl!! She's born!!
6/29/08- We meet the most beautiful baby girl in the world & the amazing mother that gave birth to her!!
7/9 - ICPC has cleared - we can go home!!
2/17/09- Finalized!!!

After dealing with infertility, IVF & a miscarriage I finally realize what God had in store for us! What a joyful time to be alive!

I had an insurance card, birth certificate and adoption decree. The guy still asked for immunization records, and I said I didn't have them on me (it's really just scribbled notes in a little book anyway).

BRING EVERYTHING!! First time, 3 years ago, just bought the BC, and all went fine. This year, did the same and they pretty much laughed at me - needed adoption decree.... I was very pissed - and did not find time to go again for a few weeks....

Go right when they open in the morning. It's really the best time -- staff are in the "best" moods (out of whatever), the line will be shorter and your wait will be much reduced. I laughed that I still had to take a number and wait -- although I did understand "it was just their process". The person was "all business" but still it took 10 minutes rather than the 60 minutes I waited the first time around. I took everything I had that fit every item I read might be needed and they actually asked for it all. Whew. Card back in 10 days. (We were only asking for name change from name at birth to the name we gave our daughter.) susan

I just went yesterday to try to get a card for my three year old adopted son, just got the bcert last week. I had to go home cause I didn't have everything:

Bcert.
Adoption decree
insurance card
**Letter from Pediatrician, stating he is a current patient, name and birthdate on it.
