Thanks Joe. I keep thinking she is ready for the toddler bed but hesitant as I worry I won't hear her get up. I already have the gate on the door and have since day one.

As for the rocking, that is exactly what I do. I just love holding an almost sleeping child.

I know exactly what your talking about with the bumping the head. I have had this issue with Nicco for several months. He also gets his limbs stuck through the bars occasionally. No solutions for you on that. As far as waking her, it could be, but it is also very normal for her to be waking several times a night with or without the head bump. Is she going back to sleep once you come in? Or are you picking her up?

She stands in the crib and cries till I come in, give her a hug and then she will go back to sleep. It usually doesn't take long but at almost 2 I really thought by now this would be over.

If it has been habit for you to go in when she cries at night she will probably continue to do that until the pattern is broke. I would wait longer before going in and gradually increase this the next few nights until she falls back asleep on her own.
She will always continue to wake which is perfectly normal, but at this time she doesn't seem to know how to go back to sleep on her own. Have you tried an automatic music player?

No, that is what I would love. An automatic CD player that starts playing her nightly lullabies upon noise in the room.

Quick update - she has slept through the night now for 5 weeks and in the Toddler bed for almost 2 weeks. We no longer rock to sleep and she is doing fantastic. Thanks again everyone for your advice.
