Social Security questions! HELP!

I know Stormster had a thread going last week but i have a question.

Ok so I am looking at this form and it says mother's full name at birth, dad's full name at birth and number etc... Does that mean me & DH or his birthmom/dad??

Also I have his adoption decree, new BC and insurance card. Is that enough??

Oh... Do I need to bring him with me to the office??

One more thing... Do I need any documents for myself??

Okay, trying to remember. I'm pretty sure I did our names, not birth parents. After all, that is what the new BC says. I think the decree, new BC and insurance card are exactly what my DH took. And nope, you don't need to take your son.

For my boys I took:

Amended BC with new names
Adoption decree with new names
Termination of Jurisdiction (saying they were no longer wards of the state) with old names
Shot records with old names
Applications

I didn't put anything in birth mother or father's name. I left it blank. I just put my boys old SS# and old names.

They have brand new SS# because their birth relatives used their SS# in the past fraudulently.

You do not need to bring him, unless you want. I didn't take my boys....It's usually pretty crowded and nowhere to sit..or at least that was my experience.

Only thing on myself I needed was a valid ID

I had to take:

Amended BC showing our names as parents
Shot record (proves identity)
Application for SSN
Certified adoption decree

We went into there to change her SSN and am still awaiting the results. We applied 2 months ago. They first wouldn't do it because we didn't have her shot record which our attorney said they didn't need. They originally told us because she "knew" she was adopted they wouldn't change her SSN (she's 9) so I went back and got a different lady whom goes "Oh let's just apply like she doesn't have a SSN" and we did that as she was afraid they wouldn't change it due to her age. Not sure what that meant but I'm still waiting to see what happened. Hoping it worked out as I don't want to keep her current SSN because her bioparents possibly have it.

That's so odd...I went in there and took everything I listed above and had the new SS# the very next week..I was shocked I got it so fast....

I know my kids b-parents have their old SS# and they HAVE used it.....

Maybe it's because of her age....

I think you are right. They hesitated changing it but after explaining that they can and my attorney advised me they can and that both bioparents are currently in prison she decided to go ahead. Then she called me a few days later saying she's mailing me a form for me to sign and mail back to her and she'll then send it off to get it taken care of. I asked them if the old #s would be canceled and she said no they wouldn't they would have 2... One in their old name and one in the new name.....I thought she was mistaken so I had her ask 2 other people. But they said the same thing.

Ok that explains why then it wouldn't matter about the passports. She could then technically have 2 right now but I just haven't received the new one yet? I'm wondering whom I should call to check on it. Seems like it's taking along time. Have any ideas? Unfortunately I didn't keep the ladies phone number whom called me after I applied.

I took Natalie's shot records, amended bc, and adoption decree. I'm pretty sure that I needed 3 forms of id for her. All I had to show for myself was a photo id (drivers license). Natalie was with me but I don't think she had to be. It only took a week or so before her new SS card came in the mail. We kept the same number since we had been Nat's legal guardians before the adoption and had used the number to claim her on our previous year's taxes.

Good luck...you will be so happy when all the paperwork is done!

Glad to hear that Supa.

I have to go this week to get our little guy's S.S.card. Does every place tell you they need the shot records? Or is that a state thing? I can't find that on our S.S. website.
