Drivers License.

That's what the SS toll free number told me.

The guy helping us told me that they send the two forms to the state so the state can verify they are legitimate, then they mail out ss card. I applied Dec. 31. Today I got a letter in the mail saying we should have the card within 2 weeks and to call if we don't receive it.

I'm finding this is different in some places.

We went in without a shot record and they wouldn't do a thing for us without it so we had to call her pediatrician, get her shot record (with her new name on it) and then go back to the SSI office. They said in order to prove identity they needed a shot record. Even though I had the certified final adoption decree that was not enough for them.

I didn't have a shot record either, but I had our insurance card, and apparently that was enough. Now, the guy was an idiot and had no clue what he was doing, and i'm still waiting for DD's SSN, while I got DS' two weeks ago...

ss card

I just went there on Tuesday...I took her birth cert. and adoption decree! I filled in dh and myself as parents and told them it was an adoption! They explained that they need to send a form to KS (adoption state) to verify the birth. I would be getting the card in about 10 - 15 days! I also needed to show my drivers license to them.

May 25, 09 - Found out birthmom was smoking crack & pot while up here! Was smoking crack the whole time during pregnancy, was stealing from stores, trying to hook out of my sisters house...wouldn't keep her blood sugar under control, etc. so we had to give her a choice...go to drug rehab or go home! She decided to go home because she wasn't ready to get off of the drugs! She was planning on taking us for everything we had (her words) if it were to be a boy!(we would of found out on 5/28) My heart is still breaking!

They asked me for a shot record. I did get my card in 7 days.

I just got back from the SS office and it was so easy! I took the original BC (has our DD's name exactly as it is now, her bmom's name, and no name for the bfather), her US passport, the adoption decree (which they only glanced at), and my own identification. I had called before going to ask about using her original BC, and they told met that if her full name had not changed, that would be okay to use. They didn't ask for the immunization record. I'm so glad that our DD's bmom offered to put the name we chose, with our last name, on the original BC. Perhaps the amended BC is only needed when the name has changed?

That could be. We changed parts of Cameron's name and needed the ammended BC.

I wonder if anyone really knows what is needed!

It takes 15 months for an amended BC here, so we won't have one until next year. But we had no trouble applying for a SS# for David either. It went very smoothly. It says right on the form you fill out that they can use the adoption decree in place of the birth certificate and we were told to insist on talking to a supervisor if they didn't want to do it - but the woman helping me didn't make an issue of it. And David's name did change - middle and last.

Along with the decree, we brought his insurance cards with his old and new names, his shot records and a copy (not original) of his original birth certificate.

We got our SS# very easily and quickly. I was surprised. Now we want to do a passport. Anyone have any advice on that?

Passport is easy as can be with SS #. The photo can be tricky esp. if baby isn't sitting up yet. Go to a place that has experience doing babies just call in advance They either lay them on the floor or do it in stroller with black cloth behind their head.

Here's a link to passport info Special Requirements for Children Under Age 16

When we got our toddler's photos, we had to take a lot of pictures. He didn't want to sit by himself so they tried photos with a sheet over me, but then it looked like a photo with someone with a sheet over them and that's not allowed. Eventually they just gave my husband the camera and we got a decent photo. We'll see how the baby's photos go.

You can make an appt for dealing with the application. That way, ,you don't have to stand in line. Or at least we could at our office. I believe both parents have to be present.
