Need example of "Permission to treat" form
Hi. I am in need of help writing a "Permission to treat" letter for us to give our foster son's birth mother.
Her and her parents are taking him camping from Friday afternoon to Tuesday afternoon. She and her parents will bring our fs to his birthday party on Sunday for a few hours and then take him back with her to the camping site. This truly scares us because she needs supervision herself. Also, our fs has only had 3 or 4 single overnight visits since he was born - and - none of them have been this year!!! She had these visits when she was living at a "girls home". Two weeks ago she was placed back into her parents care. Thanks. Christina |

I don't think you can do it, I am pretty sure it would need to come from the county, since the county would have custody.
But the form the county gives is pretty simple - like a basic permission slip Diane

Permission to Treat
It is so hard to think of a child in your care requiring treatment (whether a bee sting or broken limb!) and someone else giving the ok's.
Generally it states; To whom it may concern, *name", has permission to obtain treatment for "name" for the following timeframe (provide dates) ... and you can add "following telephone consultation with me and provide contact info ....". If you cannot reach me directly, please contact (and give a second contact) who will attempt to reach me. His/her primary care physician is "name and contact info", his/her dental provider is "name and contact info" and any medications prescribed for the child are usually handled by "name of pharmacy and contact information". I would also add birthdate and social security # ... and any recent treatment/medications as if an emergency were to occur she may panic and her family won't have all this information.
