Questions to ask before and after placement?

Hi!

I thought I saw thread here, a while ago, about all the questions you should ask about a child before and just after placement.

I remember seeing a pretty comprehensive list but I can't find it now. So, if anyone can guide me to it, or has any input, please let me know!

This is a checklist we were given in our PRIDE classes

Initial Questions
Name
DOB/Sex/Race
Grade
Reason for placement
Behavior/Medical issues
Special Ed needs (BD, IEP, 504 etc)
When placement will occur

If you take the placement ask these Q's
Caseworker contact info
Length of placement
Name and location of parents
Name and location of sibs
Who can child have contact with and how
Have times been arranged for court? visits? counseling?
Child's interests/hobbies
How does child relate to adults? peers? younger kids? pets?
Current doctor? dentist?
Church?

When the child is placed
Ask for the medical card and record folder
Clothing inventory with caseworker
Ask for clothing voucher when needed.

NO PROBLEM We got this during pride on a dry erase thing from a junior college with advertising (hoping you'd take your continuing ed stuff through them! LOL).

Are there any additional questions to ask when you go to committee for a child?

I was called today for the first time since I was licensed 3 years ago. It is an adoption placement as I do not have my foster parent license.

I ended up declining going to committee for the girls- they are 10 months apart and if I had been matched, it would have been 3 kids 18 months apart and that seemed alot to take on at this time.

With the committee call- I got all the basics on the girls- that covered all the questions in the list post above. But are there other questions I should ask, other things to try to get in terms of background?

The girl's case seem straightfoward- if there is such a thing.

If your going to committee I think in most areas (mine anyway) you can review pretty much the whole file after signing a confidentiality statement.
