letter of intent to adopt

Has anyone written a letter to court about your desire to adopt your foster child? I think its called a letter of intent to adopt. I need to write one and was wondering what to put in it. Thanks!

our caseworker gave us something to sign. i think it was a pretty generic thing the county had. then she filed it.
