Searching for a birth mother. Adoptee was listed as Baby Boy Murphy, born May 18, 1954 in Ravenna, Ohio. Adoption took place in Portage County, Ohio Feb.24,1955. There were pictures of the adoptee as an infant with other children in the home or orphanage (location unknown). It might have been a FCH. Any help would be great.

He was adopted before Jan 1, 1964 so he can copy this affadavit and follow instructions.........send it to the address on the bottom and will get his original BC and the contents of his adoption file. Enclosed are copies of two items of identification. I am aware that other items of identification may be required before the copies can be mailed. ________________________________ (Signature of Adopted Person) _________________________________________ (Street Address) ___________________________________ (City, State, Zip Code) Sworn to before me and subscribed in my presence, this _____________ day of _______________________, __________. (Month), (Year) ___________________________________ (Signature of Notary) ______________________________ (Date Commission Expires-Official Title) Section 3107.38 of the Ohio Revised Code provides that an adopted person whose birth occurred in this state and whose adoption was decreed prior to January 1, 1964, may request in writing (notarized affidavit form) and two items of identification attached to the request, may receive copies of the contents of an adoption file. Items of identification include, but are not limited to, a motor vehicle operator's license or chauffeur's license, identification card, marriage record (to provide linkage between the maiden name and married name), social security card, credit card, military identification card or employee's identification card. Effective October 25, 1996, the fee established for copies of the contents of an adoption file is $25.00 pursuant to Section 3705.241 of the Ohio Revised Code and should be made payable to: Treasury, State of Ohio. Fire Destroyed Original Records
To my understanding, the Portage County Courthouse burned down in the 50's or 60's. The records were destroyed. Would copies of the adoption records have been sent elsewhere at the time of the adoption? Did a quick search and it appears as if the current courthouse was built in the 1950's, so either the current one replaced the one that burned (in 1955 or shortly after), or there was a fire that damaged the records but didn't destroy the building at a later date.

Best way to find out is to send for your origianl BC.
See it depends on who handled your adoption? If it was handled by an agency then they have a copy too........so not to worry much about the fire OK? At least your original BC will tell you some things you don't know now.....right. :-) Blessings Mackie Quote:

I contacted Ohio Vital Statistics. She checked for me and they do have the original birth certificate on record. I was told to fill out the form you posted, have it notarized, and send it to them with payment and identification verification. Then they will send me the certificate. Easy!

If you are getting your OBC let us know when it arrives theres lots of info on there to help take you a long way towards locating birth mother and or sibs.
