What is the medical form/letter like?

As we get nearer to starting the adoption process, I am curious what the medical information for the Ethiopian dossier entails. Any info is greatly appreciated. Thanks!

You're talking about the form that we, as adoptive parents, have to have filled out by a dr, right? At first I thought you meant the medical history you receive on your child, but I think I'm reading your questions right!

We just had our medical statements done for our homestudy and it was nothing - A few yes or no questions about whether you've had substance abuse issues, if you have any communicable diseases, any heredity diseases in your family, things like that. The part the dr. filled out was just as basic.

You will also have to have one done for your dossier. Our agency gave us a form letter and we had our Dr. copy it on his letterhead four times and have them notarized. Hope this helps-Kellie

I think some of the info depends on the agency. This is fresh in my mind right now because dh and I have our physicals on Friday and I was looking over the paperwork. For our homestudy agency, we need to get physicals with everything from urinalysis, bloodtest to test for disease, blood pressure etc. The doctor has to fill out this 5 page piece of paperwork about all our medical info from infertility issues to health. For the dossier, there is a seperate form letter which the doctor has to print on their own letterhead. This one is more simple just stating that we are in good health and will be sufficient adoptive parents. As said before, this one must be notarized.
