Screening medical issues on your own?

We live in a state that does not require the use of an agency so, in addition to listing with an agency, we have posted on Parent Profiles. We have had great luck with birthmothers getting in touch with us through this site. However, I was curious (for those of you who may be doing something like this) how are you finding medical information out on the birthmothers you speak with? My lawyer can't do any screening for us so we have to handle this ourselves. I have the form our agency asked us to fill out (the check yes, no, maybe for each condition in the family or child) but sending it to a birthmother seems very cold. However, I can't get access to her medical records until we decide that she is "the one." Any thoughts or experiences on how to do this tactfully?
