Hello. All you need to do is call them, if you have the exact one it maybe more beneficial. anyway, give them a call and they will tell you what they need. My mother called them last week, (New Haven one) and they said it will cost $35 dollars. Good Luck!

I just did that from CC in Norwich CT. All you have to do is call them and if they have any non-identifying information such as medical by law you are entitled to that. If they can't help you. You may petition the court your adoption was finalized at and they will look up the information but some charge a small fee. Mine didn't. Good luck
