I know, what a scintillating topic to rope you all in, huh?
![]() My husband takes medication for IBS on an as-needed basis. He is on cholesterol-lowering meds on a regular basis (but, as a result, no longer has high cholesterol). Are these conditions that require a doctor's letter to accompany the official medical forms for our dossier? If anyone else has had experience with one or the other, and would be willing to share what you did for the medical form, I'd greatly appreciate it. Since the medical form has to be notarized, I'm thinking it's best to have the doctor include a letter to save me from having to get it in the future. Any thoughts? Hmmmm.... hard to say. Neither of those conditions are really life-threatening (although high cholesterol puts him in a different risk category), so could the doctor write something in the form itself saying "conditions controlled by medication"?
I have hypertension controlled by medication, and that's what my doctor did. My blood pressure reading was normal on my exam form, and as a bonus, my cholesterol reading is on my blood test results (done at the same time as my other tests), so they know where I stand. My agency accepted these forms without need for an additional doctor note. Others may advise you to just go ahead and get the note and honestly, it can't hurt! Probably something along the lines of "his condition is controlled by meds and he has a normal life expectancy" is all they'd need. :-) Good luck!

My husband also takes cholesterol-lowering meds. We toldthe aganecy and they said it wasn't a problem.

I take Blood Pressure medicine also, my doctor just wrote down the prescription I am taking on the medical form and said it is under control, it is also in our homestudy that I take one blood pressure pill a day
