Look at the recient posts under letter of intent. There is a great example of the letter there. I just added on how I was going to take my son to get his Cl/Cp looked at by a local hospital. Since you daughters is repaired maybe you can add that you will get reexamined. (am I right saying her cl is repaired?)

the letter of intent is probably the same thing you need. I know I had to write my LOI and give it to my agency right after I petitioned for my daughter, then for the dossier I had to do an "adoption application letter" which was almost the exact same thing, i just changed the wording a bit. Yes, her cl is repaired but there can still be ENT complications (frequent infections requiring surgery ... ) and need for scar tissue-revising surgeries, later. so the point about getting her examined or monitored is a VERY good one!
