Question dossier/notarization question

Hi Everyone!
We're in the process of gathering our documents for our dossier. Our agency has told us that it is ok to have our medical forms notarized as a copy instead of bringing a notary with us to our doctor's office to witness them signing the form. Has anyone else had their medical form notarized as a copy? Did you run into any problems with this? I was so happy to hear that this was possible since it is much more convenient, but I just wanted to make sure this won't cause any problems when we submit our dossier. Thanks for any advice!!!

Good question! I would assume that if your agency suggested you do it that way then it should be okay. What agency are you using?

That is awesome! We went through a whole lot of trouble trying to find someone to meet us at the doctor's office since that is the only way our agency will let you do it. We didn't realize that UPS stores have notary publics but we called them and they met us there. I can't believe they are letting you notarize a copy. Lucky you!! That should make things go smoother and faster.

You may want to double check. I called the Doctor's office and they had a notary right on staff! They said that they often have things that need to be notarized and so I just used their notary! But, I am sure that if your agency said it will work with a copy it should be fine!

We were told that the doctor's signature must be notarized- no copy. My dh and I use the same doctor, I found a traveling notary on line and she met us at the Dr, notarized both forms for $20. I was very stressed about it, but it turned out to be very easy.
I would def. check again with your agency, I have NEVER heard that a copy is OK before.

We had to have a notary on the spot (the medical office manager was), as required by our agency (and I assumed China)...interesting, this other suggestion about the copies.

Double check, just to be on the safe side.

i have heard it is acceptable to have a form that you sign saying that you witnessed the doctor signing, and then notarize your signature. but i also have heard that it is not acceptable in all states.

We had a notary friend meet us at the doctor's office. (They did not have a notary on staff.) We had our office visits on different days, but my husband was able to meet the notary at the office one morning after our bloodwork came in, and everything was notarized. (We have to go back to the doctor's office, though, because he did not convert my height and weight into metric. He did on my husband's, though. )

I guess i had it easy. My brother is a nortary here in new jersey so he did everything. Our agency just wanted everything copied to make it easier for us. otherwise, it would of been alot of money .

We didn't have to bring a notary with us to the Doctor's office either. We just signed a form stating that the forms we submitted were true and accurate, etc.

Since some of us have to do it and others don't, it's pretty obvious that many of the hoops we jump through are agency things and not a China things!

We did not need a notary at the doctors office

We had to get two original copies, no photocopies, of our medical forms. We took them to the notary and wrote on the medical forms on the back, "I attest that this is the signature of Dr....." and then I signed my name under that statement. It is funny how different agencies have you do different things. Go figure!- Amie

Was my doctor supposed to convert my height and weight to metric? I was never told anything about that and he did not do it. Who said to do this?? I guess it's not a big deal since we weren't told and maybe it's an agency thing...

Good gracious, converting height and weight to metric--another thing I would have started stressing out about (had I not just had our case manager review and approve our medical forms a few days ago!). I guess that is an agency thing too. Seems like that would be done when they do the translation of the medical forms into Chinese.

Anyhow, Melony, be VERY happy that your agency is allowing you to do the notarization that way! You can double-check with them to be sure you're absolutely clear on their rules, but it sounds like that should be just fine. Unfortunately we were absolutely required to have a notary on the spot when the doctor signed, and that was really tough for us since we are on a military base and a traveling notary can't just come through security onto the base! We ended up finding someone on the base to do it for us but this has really been the most frustrating and complicated part of our paperchase (coordiating the doctor with the notary, etc.) so I really envy your ability to do it the way your agency is telling you you can.

Well I did not have our Dr's notarized as a copy but usually your tax returns are notarized as a copy and your passport copies. They should state they are an exact copy of the original on them. I am a notary and this is what we have to put to notarize a copy along with the regular notary statement. Just double check with your agency and if they say yes make sure that is stated on the copy of the original.

On our form that goes in for translation, for the weight blank, it says __________kg and for the height, it says ___________m.

(Edited to change cm to m, after checking the form.)
