Non-IDing info; Born 9-29-70 (Bureau of Vital Statistics Certificate #92-45657-70). Placed by Dept. of Child Welfare in adoptive home 2-29-72. B/Mother was white, 21 y/o at time of birth. Five feet five inches tall, 125 lbs. Straight medium brown hair, brown eyes, medium complextion. B/mother had 3 siblings at time of birth ranging from ages 17 to 13 and were all in school. Was of Catholic faith, high school grad, and a factory worker. B/Father was 22 at time of birth. Five feet nine inches tall, 165 lbs. Blond hair, blue eyes, fair complextion. Had no knowledge of B/mother's pregnancy. Maternal grandmother age 53 at time of birth. I know the guardian ad litem was released on 14th of July, 1972, which is when my adoption was finalized.
The reason I know my original name was because the lawyer accidentally gave my adoptive parents the wrong set of papers. I also have some shot records from 6/28/71 and 8/71 and 10/19/71 and 10/29/71 and 11/22/71, and also a discharge paper from hospital at 3 days old but with no name or hospital name. But the first word in the hospital does look like it says Louisville. Doctor's from shot records are; Richard S. Wolf, Louis L. Kahle, 4884 Brownsboro something, Louisville, Kentucky, 40207. I have tried to access newspapers but can't access ones that old thru internet. I would have to physically go to the library. My birth certificate numbers are, file # 116 70 45657. Registrars # 11134. Registration district # 755. Primary registration District # 2275. Don't understand this, doesn't match up with birth date or adoption date but it states it was received by local registrar 10/09/70, a little after I was born. Official impressed seal was done on 11/24/72. After my adoption. I know this is my adoptive birth certificate. Just sent request to inspect adoption records to Kentucky, wish me luck
