Sibling Notification Form in AAP Check

Does anyone know where to send this form back to? I was going to send it back to the address on the enevelope the check came in.

i didnt get any, but someone a few threads down did. is the address for the aap department, if so, i think thats a safe bet. otherwise, i suggested calling the aap hotline and asking.

Ha. I was wondering the same thing.... No address to send it back or return envelope. I tried calling the hotline yesterday, and they were so busy that they weren't taking any calls for the day. Go figure!

That hotline number is horrible. I need to spare an hour to call that number.

I see the address on the check is for the County Auditor-Controller. Hmm, they sent it to me, why can't I send it back to them.
