Doctor's office won't change records

I asked the nurse at the kids' ped's office to please transfer the immunization records from Jory's book in his birth name to the one in his new name and I was told she legally couldn't do that.

Has anyone run into this problem?

What an idiot! not you, the nurse!

They should be able to do that with the adoption papers and his medi-cal card, just as long as they have record that it is the same child.

Sunshine's nurse re-did the whole thing. New card, she stamped everything again and wrote it as if it were the original. Have you spoken to the doctor about it?

Same here. Even when I showed them the new Medicaid card that had the SAME NUMBER as the old one, just his new name, they refused. Said I had to show his adoption decree. Uhhh...right. I don't actually keep that on me!

i had to show the adoption decree with the name change, and each time with each kid at each doctors it was changed...however, they didn't make us a new shot record, just crossed out the old name, and wrote the new one. lol....it has not caused us any problems so far, so that's good. if you showed them the adoption decree, and they still won't do it, ask what they need in order to "legally" do it...and go from there. good luck! isn't it frustrating that when you finally think you are done fighting for your kids, you realize you have only just begun? lol.

wow Yash, that's interesting, apparently the nurse is a lawyer too, because she knows that changing the name is illegal.......Hmmmmm.

Yeah, definitely talk to the office mgr or the Dr. personally. I had to show the court papers to have the records changed.

I tried to get the gal at the front desk to change ds's name just based on his new ss card and new medical card, but she needed the court papers. no biggie, i just brought them in another day.

I had a similar problem at our Children's hospital after finalizing even showing the adoption decree. They refused to change our status in the records from adoptive parents to just parents of Cameron, even after seeing the legal change and our final decree. I called them from my son's room in the PICU and was pretty mad. I threatened the woman with legal action if it wasn't changed. I had enough with his health issues. Some idiot with a power trip wasn't going to win.

Sorry you're going through this. Just persevere and get it changed.

I had an issue with our local Children's hospital too. My son has seen a few of the specialist's affiliated with the hospital and his dentist's office was also affliated. They forms state that if the child is adopted, an adoption decree must be presented before services are rendered. I asked if other parents have to prove that the gave birth to their child before services are rendered and was given a funny look. I refused and called their 'head office' and told them it was discriminatory to adoptive families and I would not be providing such info. I then asked if they needed my lawyers name. It isn't on the forms anymore.
