Has anyone ever heard of a case where a social worker has adopted a child who was formerly on their caseload?

I know of a social worker who started to but then disrupted once the child was in her home.

Yes, I have a family member who adopted a former foster child from her caseload. At the time of the adoption they had not been "professionally" associated in quite some time, as the adult had moved into another department and several years had passed.

One SW told me that they can't adopt a kid from their our county due to conflict of interest. Outside their county that they work in, they can.

I am not a social worker, but I used to work on a child/adolescent locked psychiatric unit and am hoping to adopt a girl who I met when she was a patient there. I know that if I were still working at that facility I would not be allowed to adopt a former patient and continue working there (especially if it were within two years of her discharge). Each agency should have an official policy regarding this type of thing, and there may also be ethical standards within the social work field that would apply.
