To begin your search you should contact the adoption agency that handled your adoption and request "non-identifying" information from your adoption file. You should also request a "Waiver for Consent to Contact" form. The non-identifying information gives the ages, marital status, physical description of the birth parents. It may also give information about aunts, uncles and grandparents. The Waiver is a form that can be used by adoptees or birthparents to give their permission for contact should a waiver be filed by both parties.

If you do not know what agency handled your adoption, or if you were a private adoption, you can contact this office:

You should have your request letter notarized before signing and mailing. It also helps to attach a copy of your driver's license and a copy of your amended birth certificate. Keep a copy of everything for your records in case you need to follow-up.

Since you have a full name have you tried the marriage records for Los Angeles County to get information from any marriage certificate? Generally that will give the bride and groom's age or date of birth, their address at the time of marriage, their occupations and the names of their parents. Also, have you requested your "non-identifying" information from the adoption agency to help you gather information about your birth parents?

My fiancee and I are planning to start a family after we are married this January. He does not want to necessarily have a relationship with his Birth Mother, but we do want some kind of medical history for our baby. I am doing everything I can to find records, but I have never done this before and it seems a little overwhelming. I tried the recommended sire above and was not able to get any search form other than for the last name.

He was born May 3, 1970 in Los Angeles. The birth certificate doesn't show a hospital name, but we know that the certificate was ammended on June 28, 1971. The State Birth Certificate Number is 104-70-10865. The attending physician was an intern named William J. Darden, MD.
