Reversed adoption

What if after an adoption is final, the birth parents or the adopted parents changed their minds mutually for some reason.
Can birth parents reverse the adoption or re adopt or go to court and say they made a mistake they want to take back their kid.
In this situation both parents mutualy agree and want to reverse the adoption legally.
