I'm in the process of getting approved for foster care. I was wondering, what type questions should I ask about a potential placement? My ages are 1 to 5.

Here is a questionaire I found that is helpful...

1. Reason child is in care
2. Has this child been in care before?
3. If child has previous care provided, has that care provider
been contacted to take the child?
4. Cause for previous placement
5. Legal Status of child?
6. Previous care providers name(s)& phone number(s)
7. Has child made allegations against previous care providers? 8. Does child have siblings?
9. If so, are siblings at home or in care
10. Child’s cultural background/identity
11. Child’s religious affiliation
12. Does child have behaviors that are inappropriate or abusive to self, adults, children or property?
13. Level of supervision needed for this child
14. Is child in need of immediate medical care or on medication
15. If yes, care & medication needed
16. Doctor’s name & phone number #
17. Other treatment providers
18. Who in child’s family may contact child?
19. Does child have visits with parent(s)? How often? Where?
20. Case workers name & phone number

We have used some and not others and have added our own questions... Hope this helps.
