We had a problem with our FS, except he was 9. He never, never, ever wiped after using the bathroom. Made for some really nasty underpants. It was suggested by some fellow posters here that I make him wash his own underpants. He did it twice and since then he's has whitie-whites!

The other suggestion is be very clear with his social worker that his behavior has changed drastically since the phone calls began. You may want to consider supervised phone calls.

The last suggestions is documentation. Take photos if necessary. I actually did this with our FS when he hid a pair of dirty underpants outside. When I found them I photographed it and where they were and made the social worker very aware of what happened.
