I don't know specific practices for New York, but it would have to be whatever legal name was given to you at birth…or the legal document wouldn’t be valid…they cant just put whatever name they want on there and viola, the adoption is done!

Its probably your legal birth name…although it may or may not be your birthmothers last name!

If you can obtain your OBC, she’d be listed.
