My SW used the China physical form to get the information she required and we filled out the basic "general health" questions. The SW form was VERY basic so she had no problems. We had a wonderful SW and she was very accomodating. We had all our visits, etc. done before our physical (We did an expedited homestudy and we were done in one week) so she held off writing the final until she received the medical form.
My Dr., too, did not do the Hep B test - said I didn't need it since I had never had the vaccination. But, we went back and got it - I had to insist.
