dumb question about diaper blowout

hi guys,

dh and I are new at this parenting thing and have a major question. dd, now 3 months old, had her first diaper blowout. we're using huggies that go up to 15lbs, and she's currently 11.75-12lbs, not too plump and 22.5 inches long. So we're trying to figure out whether she's outgrown these diapers. They look like they fit, but I'm wondering if those sizes on the package (up to 15lbs) is really true for most babies, or whether she just needs a new size. I know this sounds dumb, but we're kinda clueless. So do diaper blowouts just happen on occasion, or is that an indication of diaper size (or something else)?
