I would definitely call the county clerk from the county you were married.

I know that for my husband's birth certificate from Michigan, we ordered an authenticated copy from one of those internet sites, and it was fine, but cost more. We also ordered one directly from the county he was born in and it was not only cheaper, but a completely different certificate. So each county may have their own form.

Also, when my sister adopted, she was given a totally different marriage certificate than the type that I obtained--and we were both married in the same county in Minnesota. But hers was ordered from the state, whereas I had just walked into the local courthouse and got mine right there. So there ARE different forms/appearances.

I don't know if that helped much, but I would call the county clerk. Hope you figure it out!
