The last name is almost always changed to that of the adoptive family. Many choose to keep the child's first name when they are that old, but some do change it. Sometimes the children themselves want to change their name.
The adoptive family usually gets a new birth certificate with the new name, as mckenna said. The old b.c. is generally sealed with court records.
