My husband is adopting my daughter. this is what i have found out. Visit this web site, it has most of the forms.

you will need, i do believe for each child, forms:
PCA 301
PCA 308
PCA 314
PCA 319
PCA 321
you may not need all of them. I don't know what county you live in, but Clinton county tells me that they can guild us in this, but they can not give us legal advice. which means that they should be able to at least tell you what forms you need. Also, you are looking at about $160.00 per child.
