I'd guess it's due to him hitting his head.
DD burst a capillary last winter when she had the flu. My uncle is a nurse and he said that happens a lot with vomiting, fever or even if they throw a really good tantrum.
There's probably nothing to be concerned about but you could always call your ped just to check.
