I had to add all three of my girls to Regence BCBS of Oregon and have some familiarity with their requirements. Give them copies of Spanish and English PGN decrees and your embassy date information. Include a letter explaining the Guatemalan adoption process that includes timelines and requirements. I would also suggest you involve your employer's HR department.
