If you go to the web site for pass ports there will be a section on how to gain a pass port for a minor whom you are gardian of.
It helped me to email the pass port offices explaining the situation ( fostering) and asking what EXACTLY I needed to have- then when they replied ( next day) I forwarded the email to my SW.
You might want to print it out and take the email with you.
