My understanding is that attorneys who submit without Preapproval believe that you will get all your previos at once - including the one for no Preapproval - and they can all be delt with at the same time. (Now why they aren't going in expecting that they have all their T's crossed and I's dotted I dont' know - but then again PGN is a very unpredictable thing.)

My personal impression is that this used to be more effective than it is now. PGN seems to be dragging more and more and becoming less and less predictable. But that is just my observation based on what I read here.
