I don't have the petition with me but I think we wrote the following:

We have struggled with infertility for the past 10 years. We were able to have one daughter through infertility treatment 6 years ago. For the past 5 years, we have been trying to add another child to our family but yet again are faced with the challenges of infertility and miscarriages. We decided to change our path and look at adoption. We would be blessed to be matched with a child from China and we attest that we would love this child as he/she were our biological child.
