This site has the forms you are looking for. It is a CA web site but I am pretty sure the forms are the same. They don't say anything about CA on them.

They are on Adobe format and can actually be filled out on the computer and then printed out.

Check the laws in your county because you usually have to show proof that you really tried to make contact with the BF before the judge will allow you to place an add/notice.
