DS is almost 11.
The only real problem we've had is when a new Dr. submitted a claim to insurance with the "incorrect" MI...In cases where we have to pick one, I refuse, I put both - and then they can decide.
When I fill out forms and they has for a middle initial, I put HB (for both of his middle names) and let them weed it out...it's not my darn fault they as for initials rather than names - and who has one middle initial anymore

I love that my son has MANY options when deciding what he wants to be called...sure, he most likely HATES them all, but heck, at least he can have a choice, unlike me.
